Quality of Life Questionnaires
The IOF Quality of Life Questionnaires are comprised of QUALEFFO-41 (a quality of life questionnaire in patients with vertebral fractures) and the WRIST FRACTURE QUESTIONNAIRE (a quality of life questionnaire in patients with wrist fractures, intended to be used as a supplement to Qualeffo-41).
QUALEFFO-41 is currently available in 22 languages and the WRIST FRACTURE QUESTIONNAIRE is available in English, German, French, Greek and Dutch. In order to access the page with the different language versions, please read the user agreement and click to continue.
User agreement for the IOF Quality of Life questionnaire between (User) and IOF (updated 31.10.2005)
The IOF grants User a nonexclusive, royalty free, limited license to use the validated versions of the IOF Quality of Life questionnaire in a standard format as available on the IOF website upon the following conditions.
- User shall not modify, abridge, translate or transform the IOF questionnaire in any manner, including minor changes in the wording and order.
- User shall not reproduce the IOF questionnaire except for the limited purpose of generating sufficient copies for its own use and shall in no event distribute copies of the IOF questionnaire by any means. When User is planning a multicenter study, it should reinforce this agreement with users in all centers.
